Mike Eisenhart

Mike Eisenhart is one of Pro-Activity‘s founders, leading the firm from shortly after start-up through most of its 20+ year history. Formally trained as a physical therapist, Mike is perhaps most well known for his work outside of the clinical environment, often embedded with employers, to improve the health and performance of their populations. Mike is a sought after speaker and consultant and has won awards in the categories of innovation and societal impact for the work he has led at Pro-Activity. He is usually on the move and always working on something, which has earned him the titles: Coach, Data Geek, Ultra Endurance Athlete, Chief Executive Ox, Thought Leader, Mad Scientist and Dad....all of which he is immensely proud. Dr. Amy E. Weller
PT, DPT. Senior Partner

Amy, a Pro-Activity founding partner, has been providing clinical and consulting Physical Therapy services for 24 years. She holds an Advanced Clinical Doctorate from UMDNJ/Rutgers, a Graduate Certificate in Women’s Health Physical Therapy and spent 10 years as a Board Certified Geriatric Clinical Specialist by the American Board of Physical Therapy Specialties - one of only 42 in NJ and less than 1500 nationally. Amy’s personal mission is to educate healthcare consumers on the impact of lifestyle management as it pertains to muscle imbalances, pain and disability associated with musculoskeletal impairments. She has a special interest in the prevention of chronic disease and is a Certified Diabetes Educator. Amy finds that a deeper understanding of her patients’ needs comes from her personal experience with endurance running and swimming. Eric Eisenhart

Eric Eisenhart currently serves as the Managing Partner of Pro-Activity. An alumni of Cornell University, in his tenure with Pro-Activity he has led business development and strategic brand positioning, having been integral in the launch of Pro-Activity's movement and fitness services, employer health screenings, race and event management services, spin out of our charitable organization BaseCamp 31, the Main Street Marathon brand, and most recently it's North Carolina and Chicago operations.  Eric holds certifications from the National Academy of Sports Medicine and Precision Nutrition. Having been involved in athletics as a competitor and coach all his life, he not only believes, but acts upon the tenants that health and personal well-being are essential building blocks for optimal performance in work, play, and life.  Eric resides in Clinton Twp., NJ with his wife Amy and three children. Click HERE to listen to more of Eric's story from his episode on The Pro-Activity podcast ("The First Followers"). 

Nicholas Pfaff
CPT, CES, Partner

Nick Pfaff leads Pro-Activity's resident data team utilizing his background from Cornell Engineering to harness data & analytics that drives Pro-Activity's results focused services.  Nick Joined the Pro-Activity Team in  2005, became a partner in  2013, and has been involved in service delivery and management throughout all of Pro-Activity's services.  A former Division I College wrestler and sprint football player, Nick also utilizes his sports performance background as a trainer, corrective exercise specialist, and corporate injury & disease prevention consultant to keep a clients in the game and performing at their best.  Hobbies include trail running followed by a pint from a local craft brewery. Click HERE to listen to more of Nick's story from his episode on The Pro-Activity podcast ("The First Followers").

Dr. Justin Bagley
PT, DPT, Partner

Justin Bagley, PT, DPT is a partner of Pro-Activity Associates a prevention and population health focused practice located in Lebanon, NJ. He received his physical therapy degree from Arcadia University. Dr. Bagley has spent much of his career focused on working with employers to improve health and quality of life of their workforce populations while reducing overall injury risk and reoccurrence. Over the last decade, Justin has been integral in the development of applied prevention and health promotion best practice including the development of Pro-Activity’s 5 ELEMENTS (MOVE, FUEL, RECOVER, ENDURE, CONNECT), which have been adopted as pillars in the APHPT. Chris Gahagan

Chris holds a B.S in Exercise science and is a Certified Strength and Conditioning Specialist under the National Strength and Conditioning Association as well as a Certified Speed and Agility Coach under the National Sports Performance.  Having worked with athletes in the Hunterdon county area since 2012, he currently serves as Pro-Activity's Head Strength & Conditioning coach, and has worked with multiple county, sectional and group championship teams, as well as individual state champions.  He believes in a progressive and periodized approach to training to produce the best long-term athletic development.  In his free time he enjoys playing soccer, training olympic weightlifting, and is an avid New York Sports Fan. Click HERE to listen to more of Chris' story from his episode on The Pro-Activity podcast ("Jumping in Feet First"). 

Dr. Dana Edwards
PT, DPT, OCS
Partner

Dana Edwards began her journey into the world of PT after a love for human anatomy. She earned her BS in Health Sciences at James Madison University, followed by her clinical doctoral degree in Physical Therapy from the University of Medicine and Dentistry of NJ. Dana became fascinated with the treatment of scoliosis and further developed her understanding by becoming a Schroth certified (C1 and C2) therapist by the Barcelona Scoliosis Physiotherapy School.  She earned her Orthopedic Clinical Specialist distinction before diving into the world of prevention, completing a certification in Applied Prevention & Health Promotion. Dana has worked in a variety of settings & has mentored many students as a clinical instructor. She is active within her professional association, the APTA, having served as NJ delegate for four terms. When she's not working, Dana enjoys time with friends/family, cooking, outdoor activities, and playing ice hockey. Dr. Aaron Perez

Aaron comes to Pro-Activity by way of the Midwest. Born and raised in Chicago, Aaron then graduated as a Doctor of Physical Therapy in 2015 from Saint Louis University. He has pursued a passion for fitness and health receiving advanced training and certifications in personal training, sport physical therapy, and prevention and health promotion. When his nose is not in the books, he can be found working out and probably dancing and rapping to his favorite hip hop songs. Dr. Ali Hartman

Clinically trained as a Doctor of Physical Therapy, Ali has a deep appreciation for the resilience of the human body and the value of caring for it in all capacities. Specializing in Prevention & Health Promotion, performance, and the management of the industrial and equestrian athlete alike, Ali most enjoys helping others achieve their best, in whatever capacity that means to them. Currently Ali serves as lead of Pro-Activity's Southeast operations, and launched Pro-Activity's EQFit brand in 2019. Dr. Mark Ciolek

Dr. Mark Ciolek was born and raised in Ohio, where he received a degree in Exercise Science Education and Doctorate in Physical Therapy from The Ohio State University. Mark is driven by an interest in connecting individuals with the capacity of the human body to thrive. With advanced training in prevention & health promotion, orthopedics, and sports therapy, Mark loves to bring his clients active strategies for health. After living in NYC & building his experience with our workplace prevention clients, Mark and his wife moved back home to Northeast Ohio. Dr. Daria Oller
PT, DPT, ATC
Consulting Physical Therapist

Daria’s lifelong love of movement led to her becoming a dual-credential clinician, earning her Bachelor of Science in Athletic Training at James Madison University and her Doctorate in Physical Therapy at Seton Hall University. While in PT school, she worked as an athletic trainer in a variety of sports settings. As a self-proclaimed nerd, she then conducted youth sport injury epidemiology research at Penn State University, while also teaching kinesiology courses and serving as a clinical instructor. She has been a dancer for over 30 years, and currently trains and performs with the FutureSTEP tap dance company, in addition to pursuing projects as a solo artist. Daria is also an avid distance runner. Dr. Carley Berolzheimer

Born and raised in Chapel Hill, Carley brings her training as a Doctor of Physical Therapy and her advanced training in prevention and health promotion to the thriving Triangle area of North Carolina. With a passion for teaching others how to take an active role in their health, Carley is energized by building relationships with both individuals and her community to create a more sustainable and healthy future. Michelle LeGrand

With a professional degree in social work from the University of Texas, Michelle followed a non-linear path to her role at Pro-Activity as a Certified Personal Trainer and Senior Fitness Specialist, including counseling and case management, coaching, and directing various committees at the Peck School as well as currently serving as the Executive Director of BaseCamp 31, a 501 c 3 not for profit organization spun out of Pro-Activity in 2003.  Having completed her most recent certifications through the National Academy of Sports Medicine, Michelle's background brings a diverse and unique perspective to the team.   A resident of Hunterdon since 2007, she is deeply committed to removing barriers and promoting greater physical, mental and social well being for her clients and the greater community.  Click HERE to listen to more of Michelle's story from her episode on The Pro-Activity podcast ("Continuing the Climb"). 

Dr. Gina Sandelli

Although Gina grew up in her mother’s hair salon sweeping floors, she always dreamed of pursuing a career in the health care field. Having received her Bachelor of Science in Exercise Science, with a minor in Psychology from Rutgers University in New Brunswick, and she quickly pursued her Doctorate in Physical Therapy from Rutgers University in Newark. Although, her passion is promoting and educating on the topic of  health and wellness, which lead to her pursuing a certification in Applied Prevention and Health Promotions, Gina enjoys and has most experience treating patients recovering from total joint replacements, rotator cuff repairs, ACL reconstruction, however has also spent a great deal of her clinical time  working with neurologic conditions as well as the pediatric population.  She is a self proclaimed foodie, adrenaline junkie, and travel bug. When she is not in the clinic, she enjoys spending time with her husband, son and, large Italian family.

Dr. Taylor Forbes

Dr. Forbes rejoins the Pro-Activity team after a few years on the front lines in one of NJ's largest hospital systems.  A graduate of the University of Connecticut and Rutgers University, Taylor has extensive experience in orthopedics, and workplace prevention and health promotion, including advanced certification and training in ergonomics.  A lifelong Equestrian athlete, she, her husband and son reside in Morris County, NJ.  

Layla Darian
PT, DPT, MPH
Layla

Raised in Atlanta, Georgia, Layla moved to Philadelphia in 2019 to combine her passion for community health and physical therapy at Arcadia University. Layla completed her studies and earned her Doctorate in Physical Therapy and her Master of Public Health in 2022. Just before graduating, Layla traveled to Guatemala to provide volunteer physical therapy services to those in rural communities who had little to no access to healthcare. Recognizing a need for increased physical activity and health knowledge that would positively impact disease rates there, Layla returned to Guatemala soon after with her own newly developed, free walking and nutrition education program for women. After this experience, Layla knew she wanted to return to the States and find a job that would blend community health and physical activity, leading her to join the Pro-Activity team! Layla definitely practices what she preaches, and in her spare time, you can find her cooking plant-based meals, playing tennis, and hiking with our athletic club!

Dr. Stephanie Gentile
PT, DPT 

Steph grew up in Medina, Ohio, where she learned her love of horses from her mom. She grew up riding and showing horses, but transitioned her focus to competitive running when she attended the University of Findlay for undergrad and her Doctorate of Physical Therapy. In her collegiate running career, Steph racked up some serious accolades, including being a 6-time All American and holding 7 school records. Steph went on to run professionally with New Jersey New York Track Club and Saucony, and was an Olympic Trials qualifier (800m, 1500m), to name a few. These days, when she's not helping with EQFit,  Steph finds herself competing in the NRHA as a non-pro, where she enjoys raising, riding and showing younger horses at the start of their career. Steph and Ali joined forces through their mutual love of both horses and physical therapy.

Dr. Ryan Smith

Dr. Ryan Smith, originally from the Midwest, received his doctorate of physical therapy from Ohio State University in 2016.  In his 5+ years experience prior to joining the Pro-Activity team, he has spent the bulk of his career focusing on strength training in the older adult population, post-partum population, and individuals dealing with chronic pain. More recently, with a strong curiosity at how “business” works, Ryan invested some time working in the creative agency industry as a project and operations manager.  Eager to get back in the trenches, Ryan joins the team, servicing clients in Southern NJ, PA and DE and is excited to push the footprint further into the Mid-Atlantic region! Click HERE to listen to more of Ryan's story from his episode on The Pro-Activity podcast ("Finding our Rhythm").

Dr. Alexis Branch

Alexis has always been fascinated by the interconnection between the mind and body. This passion led her to attaining her B.A. in psychology from North Carolina State University where she was a research assistant for a lab studying the impact of computer based cognitive games on executive function in older adults and became a National Academy of Sports Medicine certified personal trainer. Following her time at NCSU, she went on to attain her Doctor of Physical Therapy degree from Winston-Salem State University where she invested her time in community outreach, tutoring her classmates, and researching the impact of sleep on older adults as it relates to socioeconomic status. Alexis is driven by the desire to empower individuals to live a life most meaningful to them without being held back by preventable health conditions. Sean Rooney

After starting her career with a degree in graphic design, Kathryn went back to school earning her Physical Therapy Assistant degree and currently serves on the Prevention South team supporting employer-clients in and around the Florence, SC region.  Outside of the work-setting, Kathryn can often be found on the roads or local trails where her four-legged friend, or training for her next Disney 5k, 10k, Half Marathon or Marathon....or knowing her.... all of them, in one weekend!  Click HERE to listen to more of Kathryn's story from her episode on The Pro-Activity podcast ("New Faces, Same Mission").

Having originally gone to school for Sports Communications to become a sports broadcaster, Stephen decided to take a step in a different direction. While working in the HVAC industry for a year, he worked to become a certified Personal Trainer and Nutritionist through the International Sports Science Association (ISSA). Always having a passion for sports and fitness, Stephen decided he wanted to make a bigger impact in people’s lives to help them achieve their fitness and personal goals. After years of playing sports like soccer, track and even ultimate frisbee, Stephen knew he wanted to give people the same opportunity to find out more about themselves like he did through his sports and fitness journey. Ken Maturo
Ken Maturo, DPT

Ken Maturo is from Poughkeepsie, New York, and received his Doctorate of Physical Therapy from Utica University. Ken gained experience in the acute care setting at New York Presbyterian Weill Cornell, where he specialized in critical care in the cardiac ICU. He then moved closer to home where he continued physical therapy in both the inpatient and outpatient setting.  With his experience, Ken provides care to a wide range of populations from athletes looking to improve their game, to individuals with cardiac and neurological conditions looking to get back on their feet. As a former collegiate football player, Ken remains active by participating in snowboarding, biking, weightlifting, and mixed martial arts.
